Start Your Journey: Becoming a Foster Parent with Envoi Associates
Having attended our informational session and learned about the rewarding journey of becoming a family model foster parent with us, you're now prepared to take the next step. Begin your journey by completing our electronic application form!
This is where you get to tell us a bit more about yourself and your family. Make sure to list 3 nonrelative references and 1 relative reference per applicant, including names, contact numbers, and email addresses. Feel free to email your Foster Home Acquisition Specialist to provide additional references if needed to accommodate all the wonderful people who can vouch for you!
Ready to get started? Complete your Commitment Letter Acknowledgement here if you haven't yet. Each applicant should complete their own acknowledgement.
Getting Started Checklist
Document Submission: Let's get the paperwork rolling! Here's what you'll need to do:
☐ Fowler’s Disclosure & Authorization Form: Complete this 2-page document for each adult aged 18+ in your home. Remember to include maiden names/aliases/other names used, and list all addresses lived at/resided in the past 7 years, including out-of-state if any.
☐ IdentoGo Fingerprints Instruction Sheet: Follow the attached instructions to schedule your appointment(s). This step is crucial for ensuring the safety and security of our foster families and the children they care for. Once completed, retain the receipt and send it via text or email to your dedicated Foster Home Acquisition Specialist (FHAS) as initial confirmation of completion. Kindly note that the associated fees are the responsibility of the applicant and are non-reimbursable.
☐ Waiver Agreement and Statement for Criminal History Checks: Complete this agreement and statement for all adults aged 18+ in your home. Be sure to sign in 2 places on the first page to give us the green light to conduct the necessary background checks.
☐ Copies of Valid Driver's Licenses and Social Security Cards: Safety first! Provide copies for every adult age 18+ living in your home. Remember, prospective foster parents must have a valid Tennessee driver's license.
Submission Deadline: The clock is ticking! Please return all completed documents within 2 weeks of receipt. Don't forget to email or text the receipt showing completion of fingerprints. We'll need those to move forward with the process.
Background Checks: After all the paperwork is in, our team will kick into high gear and finalize all background checks. Rest assured, we'll keep you updated every step of the way.
Review and Eligibility: Once everything is in order, your dedicated Foster Home Acquisition Specialist will review all the documents to ensure everything meets our requirements. This is an exciting step as we determine your eligibility to become a foster parent with Envoi Associates.
